Hospital indemnity and ACA plans: how much of a hospital bill can it cover?

We paired a real hospital indemnity plan with 2027 ACA plans in California and Colorado to see how much of a 5-day hospital stay the cash could cover.

The short answer

On 2027 ACA plans in California and Colorado, a 5-day hospital stay could cost you between $1,875 and $11,480. A real hospital indemnity plan could pay $2,200 for that stay. That's about a fifth of the cost on Bronze and regular Silver plans, and all of it on a flat-fee Gold plan.

Key takeaways

  • The cash is the same on every ACA plan, but what you owe isn't.
  • On Bronze and regular Silver plans, a 5-day stay can cost you $9,000 to $11,500.
  • Hospital indemnity covers more of the cost on plans that already charge you less.

ACA health plans limit what you pay each year, but a hospital stay can still cost you thousands. People call that "the gap." Can hospital indemnity fill it? We ran the numbers with real 2027 plans.

New to hospital indemnity? Start with the basics.

The plans we used

The hospital indemnity plan is a real employer plan from a large national insurer. It pays $1,000 when you're admitted and $300 a day starting on day two.* We've left out the insurer's name; this is an example, not a recommendation.

The ACA plans are the standard 2027 plans in California and Colorado, where every insurer offers the same deductibles and costs.*

Plan You pay first (deductible) Then you pay Most you'd pay in a year
California Bronze $5,800 40% $11,650
California Silver $4,700 30% $11,650
California Gold (flat fee) $0 $375 a day, first 5 days $9,600
Colorado Option Silver $5,000 40% $11,450
Colorado Option Gold $2,400 30% $10,450

In every state, no ACA plan can make one person pay more than $12,000 in 2027 (the out-of-pocket maximum).* New to deductibles and metal levels? Follow the links for a quick refresher.

A 5-day hospital stay

Say you're admitted for 5 days, the bill is $20,000, and you haven't paid anything toward your deductible yet this year.

Hospital indemnity could pay: $1,000 + 4 days × $300 = $2,200, if the stay is covered and the claim is approved.

Bar chart of what one person pays for a 5-day, $20,000 stay on five 2027 ACA plans, from $11,480 on California Bronze down to $1,875 on California's flat-fee Gold plan, with a line at the $2,200 a hospital indemnity plan could pay.

Plan You pay Share the cash covers
California Bronze $11,480 19%
Colorado Option Silver $11,000 20%
California Silver $9,290 24%
Colorado Option Gold $7,680 29%
California Gold (flat fee) $1,875 All of it

For example, on California Silver you pay $4,700, then 30% of the remaining $15,300 ($4,590), for $9,290. Real bills are split into hospital and doctor charges, so your numbers may differ a little.

What this means

  • The cash is the same, but the gap isn't. On Bronze and regular Silver plans, $2,200 covers about a fifth of what you'd owe.
  • Richer ACA plans leave less to fill. On the flat-fee Gold plan, the cash covers the whole stay.
  • Compare the price. If the plan costs more per year than it's likely to pay you, savings or an HSA may serve you better.

Comparing two kinds of insurance at once is genuinely confusing. A licensed agent or broker in your state can line up your health plan and a hospital indemnity plan side by side. For Marketplace plans, trained helpers are also free through HealthCare.gov local help.

Frequently asked questions

Can I buy hospital indemnity insurance on HealthCare.gov?

No. The Marketplace sells health and dental plans. Hospital indemnity is offered through work or sold directly by insurers.

Do I need hospital indemnity if I have an ACA plan?

Not necessarily. Your ACA plan already limits what you pay each year. The extra cash matters most if a hospital stay would cost more than you have saved.

Does hospital indemnity count toward my deductible?

No. It pays you directly and isn't part of your health plan. But you can use the cash to pay your deductible.
